Job Description:

"Centerstone is currently requiring that all staff provide proof of a COVID-19 vaccination by October 1, 2021, for leadership positions, and November 1, 2021, for all other staff. Position Summary: Links consumers and their families to needed resources and services to resolve their presenting concerns. Provides initial needs assessment, triage and referral services in a clinically appropriate and customer friendly manner. Continually updates knowledge of service options and community resources. Maintains positive working relationship with all community resources. Shares in the responsibility of answering mobile crisis calls and linking those clients with needed resources or next level of care. Performs UDS screens and documents results and assists with crisis counseling when needed.

Minimum Qualifications: Bachelor's Degree in Behavioral Health/Mental health/Psychology or related discipline. Experience in a mental health or addictions setting. Valid Florida Driver's License with clean driving record and current auto insurance.

Preferred Qualification: Knowledge of behavioral health payer systems preferred. Bilingual (Spanish) preferred.

Special Skills, Knowledge and Abilities: Demonstrates clinical skills in working with mentally ill, addicted and emotionally disabled populations. Experience in emergency screening and in providing case management services to the consumers and their families in an outpatient or outreach setting. Excellent telephone and customer service skills required. Ability to learn and utilize computer software. Strong ability to perform duties within specified time frame.

Essential Functions/Job Duties:
1. Reports to work as scheduled on a daily basis to perform job duties to facilitate the development of therapeutic relationships with clients and to facilitate consistent client services.
2. Provides telephone and face-to-face triage/intervention/screening with individuals and families.
3. Completes an assessment of the client's presenting problem, living situation and support.
4. Provides short-term coordination and linkage of appointments and recommended referrals as appropriate.
5. Consults with appropriate staff/team members as needed.
6. Completes all required training events and staffing meetings.
7. Establishes and maintains a positive working relationship with collaborating agencies and co-workers.
8. Documents in record of care by paper or electronic within a specified time.
9. Performs other related duties as assigned by supervisor.

Work Environment/Physical Demands: Services are conducted in the community, via Telehealth and operates in a professional office environment; can be exposed to various health conditions. While performing the duties of this position, the employee is regularly required to observe treatment environments and to use speech and hearing to interact with others in person, via telephone, using computer technology and in writing. Reasonable accommodations will be made as needed.

Position Type, Expected Hours of Work and Travel: This is a full-time position, and the expected work hours are 40 hours per week, Monday - Friday - 08:00 am to 5:00 pm - on call hours required. Occasional travel is required.
