This program is designed to develop competencies while providing each youth with the tools to begin their process of change and be successful long after completion of services.Pathways Manatee/Sarasota3657 Cortez Road, Suite 13Bradenton, FL -34210https://www.youtube.com/watch?v=H0Qtn-KpLpc&t=2s -Please follow the link for more information about this program: - https://eckerd.org/juvenile-justice-services/community-based-services/eckerd-connects-pathways/Position ConceptThe Integrated Services Manager will implement supportive community services, interventions and activities that focus on a holistic wraparound approach to each youth's needs and the needs of the family through a collaborative effort and a strength-based approach that includes identifying obtainable goals that will assist in vocational, educational, professional and personal skill building. The Service Integration Manager will provide youth with wraparound services in coordination with family, natural supports, partnering agencies, community resources, and the Juvenile Justice System with the goal of safe and positive integration in the community and reduction in recidivism. The Services Integration Manager will participate in comprehensive team planning, conduct frequent contacts with the youth, family and community resources and arrange and/or provide transportation to structured activities, mental health/substance abuse treatment, employment and appointments.Essential Functions

  • Participate in comprehensive team planning meetings to identify services needed for youth.
  • Participate in monthly treatment team meetings.
  • Identify and refer youth to leisure, recreational, and other pro-social activities as identified in the YES Plan.
  • Identify and refer youth to applicable services in the community as identified in the YES Plan.
  • Coordinate and follow up on additional services needed, including but not limited to, substance abuse and mental health. Referrals for substance abuse/mental health coordinated with the JPO. Complete referrals for identified services within 14 days of program admission.
  • Provide family with training and support services as identified on the YES Plan.
  • Arrange for and provide transportation, if required, for youth to all scheduled activities and services.
  • Arrange for and provide transportation, if required, for family to identified activities/services.
  • Complete face-to-face contacts with identified service providers in the community, at a minimum, monthly.
  • Complete phone contacts with identified service providers in the community, at a minimum, weekly.
  • Complete curfew checks as required.
  • Promotes and facilitates value-added educational and vocational services.
  • Conduct participant drug screenings.
  • Maintain ongoing communication with program staff through required reporting procedures.
  • Follow up on all the specific tasks required to assist the youth in achieving their educational and/or vocational goals and monitor/support youth's attendance and progress.
  • Develop working relationships with youth, families, prospective employers, public schools, alternative schools, vocational resources and local service providers.
  • Establish and maintain relationships with DJJ staff, local law enforcement, juvenile court judges, state attorney offices and public defender offices.
  • Maintain up-to-date youth case files and case notes on all contact and activities relating to cases.
  • Complete required tracking and follow-up data collection.
  • Instruct specific activities, provide community supervision or perform other related duties as assigned.Position SpecificationsEducation: -Bachelor's degree, from an accredited College or University, in criminal justice, social work, education, or DJJ approved related field; and a minimum of two years of professional experience working with at-risk or delinquent youth.Experience: - Seven years of professional experience or training in social services or education professional position working with at-risk or delinquent youth may be substituted for a bachelor's degree. A Master's Degree may substitute for two years of professional experience.Skills: - A sound understanding of group leadership, group dynamics, and experiential education. Records management and Medicaid processes preferred. Advanced oral and written communication skills.Physical Demand: -Must be in good physical health and emotionally stable. Must be able to move about the property. Must be able to participate in youth activities. Must be free of communicable diseases as defined by the state and to the extent that data is attainable under the law. Must be able to pass PAR evaluation of physical techniques and written certification exam.Other: - Must meet state criminal background check requirements.Travel: -Must be able to meet requirements for Eckerd's Auto Insurance and be able to drive for business purposes.
