Job Description:

Seeking a Home Health RN to provide hands-on care in the home for a group of patients using the nursing process and clinical standards and protocols.

As a Home Health RN, you will be responsible for overseeing the activities of the multi-disciplinary team that provides care to patients, monitoring the provision of therapeutic services, and ensuring that all visits are authorized and covered by physician orders.
Key responsibilities include:

• 
  
• Oversee the activities of the multi-disciplinary team that provides care to patients
  
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
  
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
  
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant
  
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor
  
  Required qualifications include:
  
  • 
    
  • BLS (Basic Life Support) certification
    
  • RN (Registered Nurse) license
    
  • Driver's license in the State of Florida
    
    Preferred qualifications include:
    
    • 
      
    • Associate's or Diploma in Nursing
      
    • 1 year of experience as a Registered Nurse
      
    • 1 year of experience in home care
