Job Description:

The Admissions Coordinator is responsible for completing all intake/referral functions accurately to assure efficient admissions, including successful management of care center incoming and outbound calls as needed. The schedule for this position isMon-Fri, 12:30pm-9:00pm. The ideal candidate will have a minimum of 1 year experience in health care, admissions, or a customer service/call center environment. Must be detail oriented, possess time management and problem solving skills, have strong computer skills, including ability to work off of multiple screen and programs at once, Microsoft Office Suite experience to include working with Excel spreadsheets. Must type at least 40 wpm. EMR experience is a plus. Primary tasks, duties, and responsibilities: 1. Promotes and practices the Hospice mission and values and follows policies and procedures of Tidewell Hospice. 2. Functions as an effective team member who uses communication and leadership strategies, time management, effective problem solving and an awareness of ethical and cultural issues while engaged in the referral and admission process for patients and families. 3. Contacts physicians to obtain additional clinical information related to diagnosis and prognosis and to request appropriate documents. 4. Works closely with admission team members to respond to requests for information about Tidewell programs and services. 5. Develops positive working relationships with Tidewell referral sources. 6. Tracks, compiles and enters accurate data on daily/monthly reports. 7. Communicates marketing and educational needs of referral sources to Access Center Supervisor. 8. Ensures regulatory and organizational compliance related to admission policies and procedures. 9. Promotes best practices that result in optimal conversion rate, productivity, and utilization of resources. 10. Maintains the confidentiality of patients, family, colleagues and reacts to sensitive situations in a professional manner. 11. Proficient use of Microsoft Office Suite. Educational/Professional: 1. Associates degree in related area preferred. 2. Minimum one (1) year experience in hospice, health care, admissions, or customer service preferred. 3. An equivalent combination of professional experience and education may be considered as meeting the Education/Professional requirements. Knowledge, Skills and Abilities Required: 1. Possesses outstanding interpersonal communication skills needed to interact effectively in person or over the telephone, as appropriate to all health care providers, about the continuum of care including basic eligibility requirements for Tidewell services. 2. Strong interpersonal skills with proven success building teams and collaborating with others. 3. Organizational skills with a drive for results. 4. Leadership skills with the ability to prioritize, trouble shoot, solve problems and work independently. 5. Ability to maintain complete confidentiality. 6. Strong computer skills; detail oriented with proven success in data entry proficiency, 10-key adding machine, keyboarding and multi-line phone use 7. Flexible, open to new ides and learning new skills. 8. Organized, self-motivated and able to communicate with all levels of the organization. 9. Able to multi-task under pressure. 10. Committed to and uses excellent customer service skills. 11. Ability to work flexible hours including weekend and holidays. 12. Possess a valid Florida drivers license and current auto insurance and able to travel within the service area.
